Summer Forms: 2012

The following information breaks all the needed forms down by age group and program. If you have additional questions, please contact the Registration Office at 828-684-2361 or email the Registrar.

All campers, regardless of program, must complete the Camper Health Form. This form was due to the Registration Office by May 1, 2012. Please send the health form to us as soon as possible for pre-screening. Be sure to check that all four pages of the Health Form have been completed and the information has been completed by a licensed physician.

All adult participants (Family Camp, Confirmation Leaders, and Chaperones) must fill out the Adult Waiver. This form will be turned in at check in.

Elementary Campers (completed grades 1-5)

Campers attending a program at the elementary school level need only to submit a completed Camper Health Form.

MIDDLE SCHOOL (Completed grades 6-8)

• Paddler's Week - Camper Health Form and USA Raft
• Rock 'n Raft - Camper Health Form and USA Raft

HIGH SCHOOL (Completed grades 9-12)

• Mountaineers and Night Owl Mountaineers - Camper Health Form only
