We have made many changes to the pricing and policies.

Here is the information you need to register for Campfirmation 2009 at Lutheridge! Please read the information below and use these links to download the needed paperwork.

REGISTRATION:
Please use the campfirmation forms ONLY for those participating in campfirmation. For other summer programs, please use the forms online or in the summer camp brochure Thank you!
If you are using regular post mail to register for campfirmation, download the form and fill in as much as possible before giving it to the parents to complete. Then collect the forms and payment to mail in together. PLEASE make sure that each congregation's registration forms (for all campers, leaders, and family members staying with leaders) come together in one envelope with the cluster name on EACH piece of paper. You must submit a separate form for each youth and adult participant. Please INCLUDE A LIST of the AMOUNT TO CREDIT EACH PERSON along with ANY CHECKS! THANKS!
If you are registering online, you will need to pay deposits or full payments with a MasterCard or Visa. Click here to access the online campfirmation registration. In the first drop down menu, select either "Summer Programs - Youth" or "Summer Programs - Adult & Family". Be sure to select "Campfirmation" in the "Sub-Category".
If parents are registering their children, make sure they know the name of your cluster. To avoid delay with your confirmation email, we recommend you check with your email provider to approve any email ending with @lutheridge.com, @lutherock.com, or @llmi.org.
No spaces are held for campers or leaders without a registration form and either a deposit or full payment. Telling your coordinator how many youth you are bringing does not hold space for you or them.
ALL REGISTRATION FEES ARE DUE THREE WEEKS
BEFORE YOUR CAMP SESSION BEGINS!
Confirmation of an online registration will be sent to the email provided on the online registration. So if you enter your email address, confirmation will be sent to you along, with any other future notices that the parent/guardian needs to receive. If you have the email address for the parent/guardian, please enter this email address rather than your own. A link to the following documents will also be included in the email confirmation:
- Camper Health Form and Waiver (this form is due to us by May 1st at the latest)
- Medication Form (bring this form to camp along with any medications)
- Camper/Parent Information Letter which includes a packing list (and what not to bring)

Those registering by the US Postal Service will get this information by the US Postal Service or email EXCEPT for the health form (unless upon request). The form has been provided above, in the summer camp brochure, and online. To be better stewards of resources, we are looking to you to help with disseminating this form out so we do not have hundreds of unused, unneeded copies of the health form at the end of the summer. The health form is a 4-page document, and all 4 pages need to be submitted by the May 1 deadline. Please be sure to send the form out to parents in a timely manner as they will need the time to meet the May 1 deadline.
Again, those registering online will get this information by email with a link to the documents listed above. PLEASE CHECK TO MAKE SURE YOUR PARENTS HAVE THIS INFORMAITON! Online registrations will not be sent a confirmation via the US Postal Service.
HEALTH FORM AND WAIVER:
Each camper MUST have a health form and waiver signed by a parent/guardian to be able to participate in camp activities.
ALL health forms must either have the Physician's Exam completed or a health exam addressing the same questions dated within 2 years of the child's camp week. We do not want incomplete health forms submitted and reserve the right to mail them back.
Please be sure all required sections are signed and complete before sending them to us. The health form and waiver must be turned in to the Registration Office by May 1, 2009. Pre-camp screening of these 3,000+ forms is a requirement of the American Camping Association (ACA). Review the forms to see if there are any special needs of which you need to be aware
and to ensure that the form is complete and signed where required. We recommend that you make copies of all the health forms before you send them in and to bring those copies with you to camp.
We cannot accept a camper into our care until the properly signed 2009 Health Form and Waiver has been screened by LLMI staff. A PDF version of the Health Form can be found at the top of this page or here.
MEDICATION FORMS:
If your campers are bringing prescription medications to camp, make sure the parent/guardian fills out the Medication Form and brings it to camp with any medication in the original container. We will not be able to accept medications in other containers. To avoid problems or delay at camp check-in, we suggest you check this prior to leaving home. The person authorized to bring the child to camp may fill out this form if a parent forgets or is not available. A PDF version of the Medication Form can be found at the top of this page or here.
ROOMMATE REQUESTS/ASSIGNMENTS:
If possible, roommate requests are honored within the same Campfirmation Cluster. Campers may not request more than one roommate. The same two names should appear on both registration forms.
This helps us to know that it is "okay" with both families for the campers to room together. Groups of four WILL BE SPLIT. If you have suggestions for campers who should or should not be housed together, please give Pastor Mary the information at least two weeks prior to your camp week.

LEADER HOUSING:
Housing for adults is on a first-come, first-served basis. PLEASE BE AWARE that space for single rooms is seldom available on-site. Please indicate if you MUST have a single room. Otherwise, a roommate may be chosen for you, if a preferred roommate is not listed. We will contact you if we are unable to accommodate your "must have" single room request so you can choose to make your own alternate housing arrangements off-site.

DISCOUNTS AND SCHOLARSHIPS:
Discounts: Discounts are now structured differently and are actually built into the 4-tiered rate structure listed above. No other discounts are offered, but we continue to offer needs-based scholarships.
Scholarships: In order to ensure all may participate in LLMI programs, regardless of financial need, scholarships are available. All scholarship forms postmarked or faxed by February 1st will be processed and parents notified of the scholarship amount by March 10th so the early rate deadline my still be met.
